Learning outcomes

The course aims to encourage students to develop an approach to the clinical and care management of people with chronic disease who will accompany them throughout their daily lives and throughout their lives. Physiopathological, clinical, care and educational aspects of some priority health problems selected on the basis of their epidemiological relevance and exemplary will be studied in depth, both in the situation of clinical instability but above all in chronicity / disability. The impact and the experience of the disease on the patient's life and on the family will be considered and the aspects of rehabilitation and palliation with respect to the symptoms of the advanced state will be explored. The course aims to:
- recognize the problems in the different phases of the evolutionary dynamics of the disease: stability, exacerbation and advanced phase
- develop care and educational skills towards the person and family in the phases of stability - instability, in transition processes, and in redefining their lifestyle, adapting to continuous therapeutic regimes, to prevent complications
- consider patient and family partner competent in the treatment plan
- promote a palliative approach in the management of symptoms and pain during all stages of disease and at the end of life.
This teaching is built on the knowledge of the first year of "General Nursing and Applied Methodology" and "Pathophysiology applied to Nursing". The contents are linked to the modules of "Therapeutic Education" and "Community Nursing" for the early discharge trend and need for support at the time of discharge; to the module "Anesthesiology and analgesic therapy".
The course is integrated with the professional laboratories scheduled for the second year of the course.


Principles of taking charge of the person with chronic degenerative disease:
- advance patient autonomy and independence and reduce the risk of disability
- advance self-management of the disease
- ensure continuity of care: transitions, resignations, treatment reconciliation
- realize the disease trajectory

Effects of hospitalization in chronicity and strategies to avoid "hospitalization syndrome".

Problems of the elderly person and / or with chronic degenerative pathologies:
- dysphagia: evaluation and management
- risk of falling: risk factors and prevention measures
- cognitive state and cognitive disorders: evaluation and management of memory deficit, language deficit, perception deficit
- confusional state and delirium
- caregiving and burden of the caregiver
- communication with the elderly without and with cognitive impairment: ageism, elderspeak

Assessment and protocol in the first 24 hours
Assessment and rehabilitation-educational approach in the post-acute phase with particular attention to early mobilization, outcomes related to disability and family needs

Diabetes mellitus
- the educational triad: drug-insulin treatment, diet and nutrition and physical activity
- self-care behaviors of acute complications: hypoglycaemia and hyperglycaemia
- acute situations requiring integrated intervention
- interventions for the prevention of chronic complications

Parkinson and Alzheimer
- assessment, definition and expressions of the main clinical manifestations
- problems and clusters of symptoms in the trajectory of disease from the pre-clinical phase to the advanced phase of the disease
- educational-care / rehabilitation and palliative approach in the various stages of illness
- family needs and caregiving

Test with closed questions with multiple choice answer and / or open questions with short answer, semi-structured questions (ex. paragraph completion, construction of a therapeutic scheme ...).
