The course aims to offer students knowledge and reflections on the community in its various forms and forms, in order to be able to work in integrated teams, implement personalized and integrated care interventions with the family and the community the user belongs to, for the different forms of need and discomfort and become an activator of collaborations and integrations between social health services and informal community networks.
Social and health services available in response to health needs in the community: home care, intermediate care, long-term care, RSA, day care centers.

The care functions in the family: criteria for assessing and defining the care burden, support strategies and services (relief posts), early identification of barnout signs, educational support for family members and private assistants
Planned discharge and continuity of care: criteria and tools for assessing patients at risk of difficult discharge, process and phases of planned discharge, activating resources and interventions to manage continuity of care
Forms of collaboration with general medicine, family and community nurses.
